There are three entrances/exits, plus a dedicated entrance and exit to/from the Waterloo & City Line:
There are three escalators plus stairs between the mainline and Underground concourses. The top of these is beside (northeast of) platform 18. The left-hand escalator is normally the one you need. There are 4 x 13 steps between the mainline and Underground concourses.
There are three entrances/exits into a rectangular building on the northwest side of York Road, opposite the mainline station and a short distance northeast of the London Eye.
The London Eye entrance is in the southwest side of the building, whilst the remaining two are off York Road, on the southeast side. We'll call the one nearest the London Eye entrance the first entrance and the other the second. Note that the second entrance is ticket only and there's no manual gate at this entrance.
The Jubilee Line concourse is off the northwest side of exit 2 of the mainline station, including an entrance from Waterloo Road. Note that this concourse is divided into two sections, the exit being adjacent to Waterloo Road, whilst the other (southwest) section is the entrance.
The entrance is off the left-hand (southwest) side of the Underground concourse, as you enter the station. You enter a gap between the ticket sales windows and the gate line then turn left (180 degrees) to the steps down.
The exit is on the right-hand (northeast) side of the Underground concourse, as you enter the station, just beyond the foot of the escalators from the mainline concourse.
